Complete the form and return via email to Ashley.Nobles@gmcnetwork.com with a scanned copy of your $20 plan deposit check. The check and plan order form should be mailed to the Montgomery, AL office of GMC. No credit cards are accepted.
Registration in SAM (System for Award Management) www.sam.gov is required. Please provide your Unique ID# on the plan order form.
In accordance with applicable regulations, Form 1095-C and Form W-2 are available to both current and former employees by logging in to ADP. These forms may also be provided upon request. Current and former employees who wish to receive a copy of either form must submit a request using one of the methods listed below:
